Write a short symptom timeline
Note when the problem began, what makes it better or worse, and how it affects walking, work, sleep or exercise. You do not need to diagnose the problem yourself.
Keep your records together
Bring available X-rays or scans, reports, prescriptions and earlier consultation notes. If you have had an operation, include the discharge summary. There is no need to arrange new imaging solely for a first visit unless advised.
List medicines and questions
Write down current medicines and allergies. Choose two or three questions that matter most to you, such as the likely next steps, treatment choices or follow-up.
Confirm the practical details
Confirm the clinic, day and time before travelling. The website form sends an appointment request; the team still needs to confirm your visit.
